By signing this consent form, I confirm that I am at least 18 years of age or have parental consent, and I understand the potential risks associated with waxing treatments. I have informed the esthetician of any medical conditions or concerns that might affect my waxing treatment. I agree to allow Smoothies Spa & Skincare LLC, DBA Sassi Senia Aesthetics, to perform waxing services with the understanding that the estheticians will take all necessary precautions to minimize risks. However, I acknowledge that there is a possibility of adverse reactions or complications, and I voluntarily assume these risks. In consideration for the services received, I hereby release Smoothies Spa & Skincare LLC, DBA Sassi Senia Aesthetics, and its employees from any liability arising from the waxing procedures. I understand that this release includes any claims based on negligence, and I waive the right to bring any legal action against the establishment or its employees for any harm or injury.
